Didn't he die in Crisis on Infinite Earths? Then again everyone who died in Crisis has wound up alive again. I mean, I'm still holding out for Kole to show up one day...
I think he even died in his original NTT arc. I don't know whether there's a complete list out there of DC characters who 'got better' multiple times but he would have to be near the top.
Kristen had a good idea for a column - characters who have died and come back without any "good" explanation. Continuity-ignoring resurrections, as it were. There are probably thousands of such instances out there . . . who's up to the challenge?

Yeah, the one in GU was an imitation, as opposed to Grodd a few months back who died in both JLA and Salvation run, in the exact same manner too
